- 1. Please bring a picture ID and an insurance card with you. Masks are required. You are allowed to bring 1 guest over the age of 12 years old.
- 2. Please be on time: If it is your 1st visit for this pregency, please arrive approximately 15 minutes before your scheduled appointment time. If you have been established with us for this pregency, please be on time for your scheduled appointment.
- 3. Call Us: If you will not make the scheduled time, because the depth and time required for an appointment, we may reschedule your appointment if you are late at all. Please call us immediately at (337) 989-9826 so that we may adjust or reschedule.
- 4. Please give a 24-hour notice if you cannot make your scheduled appointment. There is a $35 fee for failure to give a 24-hour notice. Thank you for your understanding.
